This Privacy Policy describes how Amplitude Research treats the information we collect when you visit our website, request a quote, or participate in a survey, and your choices regarding the use and processing of the information. Any personal data obtained by Amplitude Research, whether through data collection or client-provided information, is processed in a manner which ensures appropriate security and confidentiality of the personal data, including the prevention of unauthorized access to or use of personal data and the equipment used for processing. To facilitate such protections, Amplitude Research has adopted the privacy polices below along with a Written Information Security Plan ("WISP") for its employees and services providers, respectively, which is available to all Amplitude clients upon request.
A. Description of Business
Amplitude Research, Inc., offers its clients full-service market research, customer surveys, employee surveys, alumni surveys, member surveys, questionnaire design services, survey administration, data collection, data analysis, and reporting services. We also provide proxy tabulations via ProxyTab, our proxy voting service. We serve all types and sizes of for-profit and non-profit businesses, universities, governmental units, member organizations, homeowner associations, research organizations, and other entities, via online surveys, mail surveys, telephone surveys, intercepts, field research, and other methods of data collection.
B. General Data Protection
Any personal data obtained by Amplitude Research, whether through data collection, client-provided information, or other means, is processed in a manner which ensures appropriate security and confidentiality of personal data, including the prevention of unauthorized access to or use of personal data and the equipment used for processing. To facilitate such protections, Amplitude has adopted a Written Information Security Plan ("WISP") for its employees and services providers, respectively, which is available to Amplitude's clients upon request.
C. Collection of Personally Identifiable Information
Our use of personally identifiable information depends on the type of survey and the services requested by our client. When Amplitude Research administers market research surveys, access to survey takers is provided by third-party sample partners and/or select public websites. Generally, our collection of response data does not include personally identifiable information (i.e., name, residential address, email address, telephone number, or other personal identifying information of the survey taker). Occasionally, Amplitude Research might include one or more questions on a survey asking for a respondent’s contact information. This query would be included for the purpose of checking on the quality of the data by calling randomly selected respondents to verify he/she took the survey and understood the questions. These calls are made independent of the remaining collected data. In no event would Amplitude sell or share personally identifiable information with others (including our contracting client). Any personally identifiable information would be deleted within sixty (60) days of completion of data collection unless (i) earlier deletion is requested by the respondent, or (ii) it is necessary to store the personal data for a longer period of time for statistical or research purposes (subject to earlier deletion as described in (i) above). Unlike market research surveys, when Amplitude Research administers customer, employee, alumni, proxy, or other types of data collection, personal data is often collected by us as part of the survey administration process. This information can range from an email address, telephone number, and/or mailing address, to additional information provided by our client or asked as a question on the survey. Amplitude's receipt and use of such information would be for the specific purpose of performing its services specified in the service contract with our client. EXCEPT AS SET FORTH BELOW (SEE PARAGAPH F), AMPLITUDE RESEARCH WILL NOT, WITHOUT YOUR EXPRESS PRIOR CONSENT, DISCLOSE SUCH INFORMATION TO ANY THIRD PARTY OTHER THAN OUR CLIENT WITH WHICH YOU HAVE A RELATIONSHIP.
D. Information Collected Through Automated Means
When you visit our website, Amplitude Research may collect information about your Internet domain, type of browser, operating system, referral site, keyword, landing page, date and time of your visit, length of visit, and other profiling information. We use this information to learn about the number of people who visit our site, the technology used, pages visited, search terms used, and other similar information, to assist in developing changes to our website, and for business planning purposes. We may also use trusted third-party services such as Google Analytics to track this information on our behalf. Visiting our site does not by itself result in collection of your email address or other personal data.
As part of its data collection process, Amplitude Research does not typically place cookies which remain on your device. (Amplitude’s sample partners might place cookies of this kind for their own tracking or information gathering purposes). In limited situations, Amplitude might place a cookie on a respondent’s device if Amplitude is using a static survey link. The cookie would be placed to assist in preventing multiple responses by the same respondent. These situations are the exception, as Amplitude typically assigns a unique passcode to each participant and tries to avoid the use of static access links. Also, even in those limited situations where Amplitude uses a static survey link, tracking cookies are not used if survey participation is intended to include multiple responses from the same network or device. You may set your browser to notify you when you receive a tracking cookie. For some surveys, we might use a session cookie to track the survey completion time of each respondent.
E. Methods and Categories of Collected Information
Amplitude Research collects personally identifiable information through a variety of methods: (i) Amplitude’s website contact form and brochure request page; (ii) direct email; (iii) telephone calls with our sales staff; (iv) client provided data files; (v) occasionally from third party sample providers; and (vi) occasionally from responses to survey or proxy tabulation questions or in connection with the distribution of related awards for completing a survey.
The categories of personal information we collect may include: (i) name, address, telephone number (including home, cell, and/or business telephone number), and email address); (ii) professional or employment-related information (such as occupation, job title, job function, department, years of service, and office); and (iii) demographic information.
F. Use of Collected Information
Amplitude Research does not sell personally identifiable information and we will not disclose this information, except as described in this Privacy Policy or as otherwise consented to by the affected person, except as follows: (A) To service providers who are performing services on our behalf, whether engaged directly by Amplitude Research or engaged by a third party. The service providers are authorized to use and disclose the information only as necessary to perform and provide the services for which they were engaged;
(B) If we are required to do so pursuant to applicable laws or legal or court process, when we believe disclosure is necessary to prevent harm or financial loss, or in connection with an investigation of actual or suspected fraud or illegal activity; (C) To establish, exercise, or defend legal claims, including, without limitation, in order to protect the safety of an individual or to protect Amplitude’s rights and/or property; and (D) We also reserve the right to transfer information about you in the event we sell or transfer all or a portion of our business or assets. Should such a sale or transfer occur, we will use reasonable efforts to direct the transferee to use information you have provided to us in a manner that is consistent with this Privacy Policy. Following such a sale or transfer, you may contact the entity to which we transferred your information with any inquiries concerning the processing of that information.
If we are engaged by a client to administer a customer, employee, member, or alumni survey, a proxy tabulation, or other data collection service, and data collection is represented to participants as being "anonymous", Amplitude will not disclose to our client personally identifiable information which we receive or collect i.e., the results provided to our client would be limited to a collective report of findings and/or a data file stripped of personally identifiable information.
In addition to the above, Amplitude Research may use information collected or received as set forth in this Privacy Policy.
G. Storage of Collected Information
All collected information is stored on servers, computers and systems located in the United States, which equipment is directly owned or licensed by Amplitude or owned and maintained by a third-party sub-contractor located in the United States. Amplitude Research, and its sub-contractors, vendors, employees and consultants, maintain appropriate technical, administrative and physical safeguards to protect information, including, without limitation, personally identifiable information, received or collected by Amplitude Research. Notwithstanding the foregoing, transmissions over the Internet and/or a mobile network are not one hundred percent (100%) secure and Amplitude Research does not guarantee the security of transmissions. We are not responsible for any errors by individuals in submitting information. Amplitude Research cannot guarantee against the actions of any third parties with regard to a data breach including, but not limited to, breaches of our office, network, servers, computers, e-mail programs, websites, surveys, or the survey software that we use. This includes, but is not limited to, the unauthorized alteration or interception of any transmission or data.
H. Opting Out from Survey Participation
You may opt-out of survey participation by: (a) clicking on the unsubscribe link contained in any e-mail communication received from or about Amplitude; or (b) contacting our Data Privacy Representative as set forth in Paragraph J below. Upon receipt of an opt-out request, Amplitude Research will delete the personally identifiable information within 30 days of such request.

K. Children's Privacy
Amplitude Research believes it is important to protect the online privacy of children. Accordingly, we adhere to applicable laws and codes pertaining to the protection of children’s privacy, including, without limitation, the Children’s Online Privacy Protection Act of 1998 in the United States (as amended). Amplitude Research does not knowingly collect personally identifiable information from anyone under the age thresholds prescribed by applicable laws and codes for the collection of personally identifiable information from individuals without parental consent. If we become aware that we had inadvertently collected personally identifiable information from anyone under such age thresholds, we would promptly delete such information.

Amplitude's website(s) may contain or provide links to other websites for your convenience and information. These websites may operate independently from Amplitude Research. Linked sites may have their own privacy notices or policies, which we strongly suggest you review if you visit any linked websites. M. FOR RESIDENTS OF CALIFORNIA, VIRGINIA, CONNECTICUT, COLORADO, AND UTAH, AND OTHER STATES WITH RELEVANT CONSUMER PROTECTION ACTS:
Pursuant to your applicable state’s privacy laws and regulations (the “Applicable Law”), including the California Consumer Protection Act of 2018 (“CCPA”), and subject to certain exceptions and limitations, you may contact Amplitude Research to exercise the rights described below with respect to certain personal information that Amplitude might temporarily collect about them. To the extent those rights apply to you, they are described below. Amplitude also frequently handles certain personal information on behalf of its clients. You should contact those clients to exercise any rights you may have with respect to that personal information.
Right to Know About Personal Information Collected, Disclosed, or Sold: You have the right to request that we provide you with details about the personal information we collect and use. Other than the exceptions set forth above (see Paragraph F), Amplitude Research will not disclose your personally identifiable information to anyone other than our client who contracted for the survey or proxy tabulation service in which you were a participant.
You can submit a verifiable consumer request by contacting our Data Privacy Representative as set forth in Paragraph J above or by calling our toll-free number at 1-877-225-7950, extension "0". Amplitude Research reserves the right to verify your identity to our satisfaction.
You are entitled to receive the following: (i) The type of personal information we collected about you in the preceding 12 months; (ii) The sources from which that information was collected; (iii) The business/commercial purpose for the collection or such information; (iv) The identity of the third party for whom such information was collected; and (V) The specific pieces of personal information collected about you (subject to some exceptions).
Right to Request Deletion of Personal Information: You have the right to request deletion of the personal information Amplitude Research has collected about you (subject to some exceptions) by contacting our Data Privacy Representative (see above). We reserve the right to conduct the verification described above.
Right to Non-Discrimination for the Exercise of a Consumer's Privacy Rights: You have the right not to receive unlawful discriminatory treatment by Amplitude Research for the exercise of your privacy rights under Applicable Law including the CCPA.
Types of Personal Information Collected: Amplitude Research may collect personal information from research participants during a survey, in connection with the distribution of rewards, or from lists of employees, customers, alumnae, members, etc., provided by a client to allow us to perform services for that client. The categories of personal information we collect may include:
name,
address(es),
telephone number(s) (including home, cell, and/or business telephone numbers),
email address(es),
professional or employment-related information (such as occupation, job title, job function, department, years of service, and office), additional content/material including photos, videos and/or similar content, and
demographic information.
All of the information above would be collected for the purpose of survey research or proxy tabulation services requested by our client. Other than the exceptions listed in Paragraph F above, Amplitude Research will not use this information for other purposes.
N. European Union ("EU") Data Subjects
Amplitude (i) does not have offices outside of the United States, (ii) does not maintain survey panels of persons residing outside of the United States, and (iii) does not monitor the behavior of EU subjects. However, from time-to-time, Amplitude might process and temporarily hold the personal data of subjects residing in the EU as part of its commissioned role in data collection for its clients (such as acting as a data processor for a customer or employee survey). At no time will Amplitude agree to collect "sensitive" personal data from EU subjects i.e., data revealing racial or ethnic origin, political opinions, religious or philosophical beliefs, trade-union membership; data concerning health or sex life and sexual orientation; genetic data or biometric data; or personal data relating to criminal convictions and offenses. In those rare instances where Amplitude does collect non-anonymized personal data from EU subjects, Amplitude will maintain a record of its data collection activities including field period, date of delivery of personal data to a client, and date of deletion of personal data. As a general rule, all personal data collected from EU subjects is deleted from Amplitude’s database within thirty (30) days of completion of data collection unless it is necessary to store the personal data for a longer period for statistical or research purposes. If personal data is collected for a given study, the survey invitation sent to participants for that study will describe in detail the contact procedure for a EU data subject to withdraw consent to use of his/her personal data and/or request a copy of his/her personal data used in the study.
O. Actions by Third Parties
We cannot guarantee against and shall not be held responsible for the actions of any third parties with regard to a data breach including, but not limited to, breaches of our office, network, servers, computers, e-mail programs, websites, surveys, or the survey software that we use. This includes, but is not limited to, the unauthorized alteration or interception of any transmission or data.
P. Changes in Privacy Policy
